I am getting very frustrated with our doctors surgery. We registered Babybug at our docs the day we brought him home using his birth name and when we got AO and received his adoption certificate we changed his name with them. However despite us changing it a year ago the only record they have of him is his birth name which rightly so is comin up locked. DH has spoken to them twice regarding it and complained about it yet they still have not done anything!! We know the NHS have the correct details as he is under podiatry and everything comes through fine for that and also for HV so why oh why can the doctors not get it right? Anyone else had any similar issues?

oo not good - our doctors were pretty good on this and seem to be able to coordinate pre and post order stuff.

We had thought it had all been done as correspondence from HV and also podiatry which he is under come in the correct name. It only came to light about 6 weeks ago when we booked for him to have his flu vaccination so DH explained the situation and how we weren't happy about etc, etc...and they told him they would rectify it, yet last Friday when he attended an appointment he thought he was just check and that is when they told him they only had the record in birth name. So now we have to contact the practice manager to get it sorted out as apparently she is the only one who can do it!! So frustrating!

Hi Ladybug, I'm a Practice Manager at a doctor's surgery. Please speak to the Practice Manager at your surgery. Babybug should have been issued with another NHS number at the time the AO was granted. This happens automatically. His old medical record will be archived within the NHS (tip - ask for a copy of these notes as soon as you can before they "disappear" so that you have a record just in case) and you will need to register him again in his new name with his new NHS number (for NHS purposes he has become a different person). Many of the back office functions within Primary Care have been centralised (and outsourced) recently and in our area registering new patients is a huge issue. There are many delays and errors being made and not enough people in the central department to deal with it so it may not be the fault of your own surgery. If you continue to have problems contact your local PALS; they have a lot of clout and will follow things through on your behalf. I hope this helps xx

Monkey, can I just ask please: what requirement is there for the GP surgery to redact paperwork they have from the old paper file being kept on the new file? I've only just got the new NHS no after pushing. The Practice Manager herself said that the old name should not appear as LO could ask for a copy of her record when she's older and see old name, birth family names/address etc but now, she's back-pedalling (probably because there would be a lot of paperwork to redact). I don't understand why they would keep that information when a new NHS is issued and the record attached to the old number locked specifically to avoid that situation...!!! I've emailed the PCSE to see what they have to say. (Thankfully, school have readily agreed to redact to avoid any risks of breach, especially by High School age if Kitt is struggling and an inexperienced/uninformed staff member thinks they are helping by giving her answers from paperwork rather than discussing it with me).
Ladybug: I received my letter today from the NHS confirming that Kitt has been registered with GP, with new number - so I'm hoping that the GPs don't make the same mistake as you're experiencing.

Knight, that's an interesting question and I'm not sure. Third party information would be redacted from her medical record should she request to see it (or have copies) in the future.
None of our LO's medical history prior to adoption is included in her new record and that is why I requested paper copies before the formal adoption went through. None of the third party information was redacted from this although it should have been.
I suspect that there are different procedures in different parts of the country sadly.

Knight: we have never received a new NHS number! We received a letter from the docs a few days ago to request proof of BB's new name and confirmation of his address despite the fact that we provided them with a copy of his adoption certificate when we originally changed his name with them or at least thought we had!
Monkey, am I right in assuming that a copy of BB's adoption certificate would be sufficient as not only does it confirm his name but has our address on it too?

Oh Ladybug, that's strange. PM me if you want the contact details to chase the NHS number up. My GP didn't do anything as it had to come from another body (I had given GP the AO and copy new birth certificate but they just held it on file until they were told formally by the relevant body). x

I didn't have to do anything to get my child registered. All was sorted without any involvement from us, Nicest thing was the phone call from our GP - who was v excited for the 3 of us & sent her congratulations - just as we sat down for a 'welcome' cake & a drink.!

The saga continues!! Thought it was all sorted, visited docs last week to get a referral for a hearing test for BB and it was all booked in his new name, yet today I receive a copy of the letter that was sent back in December asking to take in proof of his adoption and current address!! So I phoned them and asked what else I could possibly take in as I had already taken in his adoption certificate TWICE and second time in December the adoption order, which they didn't need as the adoption certificate is sufficient. The lady I spoke to told me there was no record of the letter on the system and that the adoption certificate is sufficient and she would email her colleague who had sent the letter to tell her we had presented his certificate twice already!! I GIVE UP!

It sounds like a complete circus! You must share the same LA as me. The first time I took Tsunami to the Doctor after our adoption order, the GP was very surprised to see an 8 year old come through the door - when they re-registered him they had put his date of birth as the date of the adoption order so he was expecting a baby!
