My ED is due to have a mri scan under GA soon, and I had to complete a telephone 'pre-op' assessment. As part of this, the nurse asked me about birth history and so I mentioned that she was adopted, but told her what I know about the birth. She then went on to say that I needed to bring adoption cert with me.
She sounded slightly embarassed when I quizzed her on why, (my feeling is to prove PR), and fudged the answer when I asked whether she would regularly ask parents to bring a birth cert.
Has anyone else come across this?

? sounds slightly odd...
I took George to A and E recently (pre adoption order) and had to provide SW details as he was still a LAC... but have never been asked for birth certs for the other two.
All medical history should be linked up even though AC get a new medical no after AO, if they want birth info you could say you could bring red book, that has more relevant info than birth cert and for proving parental rights, should you have to do this, surely all her details and next of kin/ parents are on the system?

Very odd.
No never had anything like that and mine have had loads of medical appointments and hospital visits between them.
Only a couple of weeks ago I was asked re one dd's birth history and had to say she was adopted.
I find they just either assume or check(verbally) I am mum and that's that.
Perhaps this person doesn't realise it isn't necessary??

Definately NOT needed. Birth mums don't have to take birth certs. I think she is thinking of foster care & adoption as something akin to each other & isn't sure if you have legal responsibility. Training issue for hospital team I think!

Umm, that's what I thought. In the event, it was the least of our worries. Not mentioned at all. But she was too nervous and couldn't manage the anaesthetic. Did however meet the play therapist (which I had previously asked about and was told they didn't have one!). Need to make a bit of a plan and try again.

Can i ask what the red book is? We had a small book which was filled put by drs whenever ds2 saw doctor or attended hospital, during his time as lac, is it aomething of that sort ( his wasnt red, mainly blue actually) ds1 never had anything unless his foster carer kept it,

Yes Red books are small books where child developments checks & consultations with health visitor are recorded. They belong to the parent & are usually taken to appts with HV/GP/hospital. They are mainly for pre school. They may not be red in colour where you live. BM may well have kept it! If you need one then your HV may be able to sort one for you.
